We will make every effort to place reminder calls regarding upcoming appointments, but due to the large volume of patients and limited staff, reminder calls will not always be possible.

Please note that fragrances, chemicals, and smoking are not permitted in our offices due to the number of chemically sensitive patients treated. Please help us help them by being fragrance- and odor-free when you come to see us!

Insurances Accepted: We do not accept any commercial insurance at this time; however, we do offer excellent discounts for self pay! Please call for further information.

an active referral from your base PCM)
Some insurance companies and/or insurance plans may provide for out of network benefits. This means they may pay a portion of your medical expenses, even if the doctor is not participating with their company. We will provide you with a form you can submit to your insurance company (self-file) to request reimbursement. Your individual plan will determine your reimbursement eligibility and that is solely between yourself and your insurance company. To find out more about this, please call your Customer Service number for your insurance carrier. This number is usually listed on the back of your insurance card.
We are not in network with any ObamaCare (Affordable Healthcare Act) plans, regardless of issuing company.

Directions: We are located in the Madison Park Office Park on the corner of Bayou Boulevard and 12th Avenue. Our office is easily accessed from either street. We are directly across from Assembly of God church and just down the block (on the other side of the street) from Target. We are approximately one mile (same side of the street) from Cordova Mall and Sacred Heart Hospital which are located on corners at 9th and Bayou.
